BRILLIANT HEALTHY SMILES
PATIENT MONTHLY COMMITMENT FEE - Contract
This Monthly Commitment fee (“MCF”) defines the terms under which a patient (“Member”) joins the Brilliant Healthy Smiles (“BHS”) Membership Plan. By enrolling in the plan and paying the monthly Membership Commitment Fee (“MCF”), the Member agrees to the obligations and entitlements as set out below.
1. Monthly Commitment Fee (MCF)
The MCF is £9.95 per month.
Payment of MCF allows the Member to access dental treatment by BHS on NHS pricing standards.
MCF must be paid on time for the Member to remain eligible for NHS Standard pricing treatment.
Under 18’s get free treatment, once registered to the MCF.
2. Contract Period
Upon enrollment, the Member enters into a 24-month Agreement with BHS.
The Member agrees to pay the MCF for the entire 24-month duration.
Early cancellation is not permitted except under the exceptions listed in Section 10.
3. Eligibility to Access NHS-Standard Prices Treatment
The Member will receive NHS-Standard pricing treatment only while MCF are fully paid up to date.
Missed or failed payments will result in suspension of dental treatment as described in 3.1.
BHS reserves the right to verify membership status before any appointment and treatment is offered.
4. BHS Services
Membership includes the right to:
Book appointments at BHS
Receive NHS-Standard dental pricing for eligible treatment
Attend routine check-ups
Cost for the clinically necessary treatments will be priced according to the NHS-Standard Bands for primary care.
5. Exclusions
Followings are NOT included in the MCF:
Cosmetic or private-only services
Specialist treatments
Missed appointment fees
Any treatment provided outside BHS
6. Payment Terms
Payments are collected via direct debit or another approved recurring method. MCF secures your registration with BHS Dental. It also helps BHS to extend its services to an increased number of patients and be able to accept members' appointments, even during high demand periods.
Late or failed payments may result in:
Suspension of membership benefits
Cancellation of appointments
Payments must be kept up to date to maintain access to NHS-Standard pricing treatment.
7. Member Responsibilities
Keep payment information accurate
Attend scheduled appointments or provide 48-hour notice for any changes.
Follow clinical and safety guidance as informed by BHS
Update BHS with any changes to personal contact information and Medical Information
8. BHS Responsibilities
Provide NHS-Standard pricing treatment to eligible Members
Deliver clinically appropriate care
Keep patient information confidential
Notify Members of any changes to the plan with reasonable notice
9. Early Cancellation of Subscription
Membership cancellation under the 24-month term is permitted only if:
The Member emigrates.
A long-term medical condition prevents dental treatment
BHS agrees to terminate.
Evidence may be required to approve the cancellation. Passing of a member must be notified to BHS as soon as possible to update its records.
10. Renewal
Following the initial 24-month term, the membership automatically renews for further 12 Months period unless the Member gives 30 days written notice of cancellation prior to the expiry of 24 months agreement.
11. Data Protection
BHS complies with GDPR and UK data-protection laws.
12. Governing Law
This Agreement is governed by the laws of England and Wales.
Member Declaration
I confirm that:
I have read and understood this Agreement.
I agree to pay the Monthly Commitment fee of £9.95 for the continuous period of 24 months.
I understand that treatment of children under-18 is free while the MCF payments of their parent/guardian are up to date.
I understand the MCF will receive NHS-Standard pricing treatment and costs.
